10th Anniversary Dynamic Decade 2014-2024The Thanksgiving season is upon us, a time to be thankful and grateful for the bounty in our lives. It is also a time of heightened loneliness for many people. Chicago Hyde Park Village is making a change! CHPV, Augustana Lutheran Church, and the Hyde Park community come together on Thanksgiving Day for our 12th annual Community Thanksgiving Day Meal and to deliver Thanksgiving meals to dozens of older homebound neighbors who are unable to enjoy the community gathering in person. I'm thankful for the generosity of neighbors helping neighbors. Will you help us "Make a Change" this holiday season?

Advanced Care Planning Informational Seminar
Tuesday, December 5, 3-4pm

Join CHPV, UChicago Medicine, and the SHARE Network for an information session on Advanced Care Planning! Advanced care planning is the process of preparing for future decisions about your medical care if you become seriously ill or unable to communicate your wishes. Participants will have the opportunity to sign up for one-on-one consultations with a SHARE Network team member in January. Participants will have opportunities to complete advance care planning documentation and share them with family members and/or healthcare providers. Light refreshments will be provided.
